Nyansapo Healthcare Group is launching a Community Health Screening Initiative to strengthen preventive care, promote early detection, and expand access to basic health screening services in underserved communities across Ghana.

We invite eligible health institutions to apply for partnership under this initiative.

 Key Information

• Application Deadline: March 31, 2026
• Estimated Completion Time: 10–12 minutes
• Submission Limit: One application per facility
• Eligibility: Government, mission-based, and licensed private health facilities serving underserved or high-need communities

 Before You Start the Application

To complete the form efficiently, please have the following information ready:

 Facility Details

• Official facility name
• Region and district
• Type of facility (CHPS, Health Center, Polyclinic, Hospital, etc.)
• Estimated catchment population
• Average monthly patient volume

 Community Health Profile

• Most common health conditions seen
• Current screening activities (if any)
• Distance to nearest referral hospital
• Key underserved or vulnerable groups

 Operational Readiness

• Number of clinical staff available to support the event
• Availability of space to host the screening
• Ability to mobilize community members to attend
• Basic infrastructure readiness

 Follow-Up & Reporting

• Ability to provide follow-up care for abnormal findings
• Established referral pathway (if applicable)
• Willingness to share aggregated, non-identifiable screening data

Note: Nyansapo will only request summary data such as:

• Total number screened
• Gender distribution
• Age categories
• Percentage with elevated blood pressure or glucose
• Number referred

No patient names or personal identifiers will be collected.

 How Applications Are Evaluated

Applications will be reviewed using a structured scoring framework that considers:

• Community health need
• Screening gaps
• Institutional readiness
• Follow-up capacity
• Expected impact

Facilities serving remote, underserved, or high-burden populations with strong mobilization and follow-up capacity will be prioritized.
